Health Care Survey
In order to highlight Economic Human Rights violations around health care, WEAP has created a health care survey that aims to collect information on your own personal experiences with the US health care system. Why? Because workers all across the country, employed and unemployed alike, face increasing difficulties in accessing decent health care. The survey only takes a few minutes to complete and the results will be publicized in order to help expose the millions of Economic Human Rights violations occurring daily. To take the survey, you can do one of the following:

2. Or you can download our Economic Human Rights Violation Report Form and Health Care Addendum here. Fill these survey forms out and then fax, mail, or walk them into our office.
